Store Health Plan

The Store Health Plan is available to all regular full-time employees.   The Plan comes with in-network  benefits only.   

The Plan, administered through United Health Care, provides health and prescription coverage that works the same for most services. First, you generally pay an annual deductible. Then, you and the plan share the remaining expenses on a percentage basis. The plan includes coverage for all kinds of medical expenses, such as inpatient, outpatient, office visits and preventive care.   It also includes prescription coverage, which has a separate co-pay.  

The Plan comes with an office visit of $10 which means that all you have to pay when you go see an in-network physician is $10. The Plan has a calendar deductible of $400 per person and a family calendar deductible of $800. The plan covers all eligible expenses at 80% after the deductible and you pay 20%. The Plan has a $10,000 per year maximum per covered member.

Prescription Card Service
The plan comes with a Prescription Card Service that allows you to purchase prescriptions for a co-pay. The prescription drug service is administered by Medco. Under the prescription drug options, the amount of your co-pay depends on the type of prescription. The cost of prescriptions is: tier 1 - $7; tier 2 - $30; and tier 3 - $60.

Medco Mail Service Pharmacy
Medco also offers you the advantage of ordering your prescriptions by mail. The Mail Service Pharmacy is open 24 hours a day, seven days a week to take prescription orders and answer questions. Through this service, you save time and money on medications you and your covered dependents take regularly. You can purchase a 3 month supply of medications for the price of 2 ½ months and get them delivered directly to your home at no charge to you.
